Welcome to Oklahoma Health Network (OHN). OHN currently has more than 6,000 contracted provider locations across Oklahoma with new providers are being added daily. If you do not see your physician listed in the Provider Directory, please let us know by completing a Provider Nomination Form, which can be found above. This form can be completed and returned electronically or you can print a copy and fax it to 405-440-8847. Please be sure to include your contact information so that we can respond to you regarding your request. You may also want to contact your provider directly to let them know you want to continue to see them and request that they participate in OHN.
